LPN - Licensed Practical Nurse
Job Summary:
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Advocate Healthcare of East Boston. Responsibilities:
Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities
Develop and implement individualized care plans to meet the unique needs of each resident
Collaborate with the interdisciplinary team to identify and address resident needs, concerns, and goals
Conduct routine assessments and report findings to the nursing team
Maintain accurate and thorough records of resident care and services
Support and educate residents, families, and caregivers on healthcare practices and best practices
Participate in quality improvement initiatives and staff meetings to continuously improve the quality of care and services
