RN Hospice Registered Nurse (PRN)

On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience. Responsibilities Performs initial nursing assessment on new admissions. Completes , coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff. Regularly communicates patient progress to the clinical manager, physician, and care team. Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies. Records pain/symptom management changes/outcomes as appropriate. Educates families on patient condition, needs and care. Makes referrals to other hospice care disciplines as needed , or documents rationale for not doing so. Performs other duties as assigned. Qualifications Required: Current RN license in the state(s) where you will work Minimum 1 year of clinical RN experience in an acute healthcare setting Current CPR certification Preferred: Hospice or home health experience Strong communication and interpersonal skills Ability to work independently and manage multiple priorities Comfortable with daily travel between facilities Proficient in electronic medical record (EMR) systems This position requires a background screening through the Florida Care Provider Background Screening Clearinghouse.
