This grant-funded Care Coordinator position plays a central role in expanding access to medication for opioid use disorder (MOUD) and helping clients successfully navigate treatment, recovery, and supportive services.

As the Ocean County program transitions from a telehealth-only model to in-person buprenorphine and extended-release naltrexone treatment, with on-site methadone dispensing planned as the program develops, you'll help ensure clients receive timely access to treatment and the support they need to remain engaged in recovery.

What You'll Do

The Care Coordinator supports enrollment, intake, warm handoffs, retention, follow-up, and care coordination for clients receiving medication for opioid use disorder and related behavioral health services.

Intake & Clinical Support

• Complete intake and screening, including validated substance use and co-occurring mental health screenings.

• Complete military service history screening at intake.

• Support DSM-5-TR assessment, Addiction Severity Index, biopsychosocial evaluation, and Level of Care Index administration.

• Ensure ASAM Criteria placement is documented and revisited during reassessment.

• Verify PDMP review for every new admission in accordance with applicable requirements.

• Support timely MOUD initiation for clinically eligible clients, with a program goal of initiation within 72 hours of intake.

• Help clients transition from telehealth services to in-person induction, observed dosing, and wraparound clinical services.

❤️ Client Engagement & Retention

• Provide retention-focused case management.

• Conduct appointment reminders and follow-up with clients who miss appointments.

• Perform outreach and re-engagement activities to help clients remain connected to treatment.

• Support annual reviews of treatment and recovery goals.

• Support continued medication engagement and provide intensified monitoring when clinically indicated during medication tapering or discontinuation.

• Build trusting relationships with clients using a trauma-informed, recovery-oriented approach.

Care Coordination & Community Resources

• Connect clients with co-occurring behavioral health treatment and recovery services.

• Link clients to housing, recovery housing, peer recovery support, benefits, employment, education, primary care, and legal resources.

• Confirm linkage to care for clients who screen positive for HIV or Hepatitis C and document closed-loop referrals.

• Coordinate with the VA New Jersey Health Care System to help eligible veterans access VA benefits without interrupting MOUD treatment.

• Assist uninsured and underinsured clients with Medicaid and NJ FamilyCare enrollment.

• Coordinate services with healthcare providers, community organizations, and other members of the treatment team.

???? Contingency Management & Recovery Support

• Deliver contingency-management incentives tied to documented target behaviors.

• Follow applicable SAMHSA motivational incentive requirements and program procedures.

• Maintain accurate incentive logs and supporting documentation.

• Encourage engagement in treatment and recovery-oriented activities.

???? Data Collection & Follow-Up

• Administer required SPARS client-level data collection at intake, six months post-intake, twelve months post-intake, and administrative closeout.

• Submit collected data within required timelines.

• Conduct outreach to locate and re-engage clients for required follow-up assessments.

• Review project data for completeness and accuracy.

• Support program performance goals related to MOUD initiation, retention, and follow-up completion.

???? Documentation & Compliance

• Document all client contacts and services in the Electronic Health Record within required timeframes.

• Maintain accurate, timely, and confidential client records.

• Obtain and maintain valid consents for disclosure.

• Follow 42 CFR Part 2, HIPAA, and organizational privacy and security policies.

• Follow all applicable federal and New Jersey requirements governing opioid treatment services.

• Participate in required trainings, quality improvement activities, and program meetings.

???? Transportation & Community-Based Services

• Operate an agency fleet vehicle to transport clients to appointments and dosing as needed.

• Travel extensively throughout the Ocean County service area.

• Provide services in outpatient, office, and community settings.

• Maintain a valid New Jersey driver's license and meet agency driving and insurance requirements.

QualificationsRequired Education

• Bachelor's degree in Social Work, Counseling, Psychology, Human Services, or a related field

OR

• Associate degree plus three years of relevant case management experience

Required Experience

• Experience providing case management services in substance use disorder or behavioral health settings.

• Familiarity with ASAM Criteria and levels of care.

Preferred Qualifications

• CADC

• CADC-eligible

• LCADC

• Current Certified Peer Recovery Specialist credential

• New Jersey Certified Community Health Worker credential

• Experience serving individuals receiving MOUD

• Experience working with individuals involved with the justice system

• Bilingual English/Spanish

Credentials

Candidates must be eligible for Certified Peer Recovery Specialist or New Jersey Certified Community Health Worker credentialing and must be willing to obtain the appropriate credential within the timeframe established by IRON.

Additional Requirements

• Must possess a valid New Jersey driver's license.

• Must maintain a motor vehicle record satisfactory to IRON and its insurance carrier.

• Must remain insurable under agency policy.

• Must successfully complete required background checks.

• Must maintain all required licenses and certifications.

• Must complete required onboarding and annual training.

• Must comply with 42 CFR Part 2, HIPAA, and all applicable confidentiality requirements.

• Ability to travel throughout the Ocean County catchment area.

• Ability to operate agency fleet vehicles and transport clients.

• Ability to sit for prolonged periods and use a computer regularly.

• Ability to frequently stand and walk and occasionally lift up to 25 pounds.

• Ability to work in outpatient clinical, office, and community environments.

• Must follow required infection-control and personal protective equipment procedures.
