Your Role The Executive Director of Population Health leads the systems through which Near North Health organizes and improves care across the population it serves. The role is cross-cutting by design. It spans operations, clinical programs, information systems, managed care, care coordination, and data, and it exists to connect these functions into a coordinated approach to the health of the whole panel rather than the care of one patient at a time. As a member of the executive leadership team reporting to the Chief Executive Officer, the Executive Director owns the value-based care model, the population health infrastructure, the care coordination function, and the cross-cutting programs that close care gaps at scale. Compensation Compensation is determined based on each candidate’s experience, skills, and education, within the established range for the role.
