Work Environment

This position is primarily onsite, allowing you to work directly in the homes of our patients, which offers a unique setting to form meaningful connections and provide personalized care. Our home care model highlights the importance of treating patients where they feel most comfortable, ultimately promoting better recovery outcomes. You will have a flexible work schedule from Monday to Friday, with hours typically from 7:30 AM to 4:00 PM or 8:00 AM to 4:00 PM, ensuring a work-life balance.

Key Responsibilities

• Conduct thorough evaluations of patients in their homes, assessing their physical abilities and limitations.
• Develop and implement tailored treatment plans that utilize a diverse range of therapeutic techniques, including exercise, stretching, and hands-on modalities.
• Document patient progress accurately, tracking treatments and outcomes to inform ongoing care decisions.
• Reassess patient needs and adjust treatment plans as required, ensuring that goals are met effectively.
• Provide instruction to patients and their families or caregivers on exercises and assistive devices that facilitate self-care and mobility.
• Engage in community initiatives and workshops, promoting health education and preventive care.
• Maintain professional development through continuing education opportunities and participation in industry networks.

Patient Population / Client Focus

Our Home Care Physical Therapist will work with a diverse patient population, including children, adults, and seniors recovering from injuries, surgeries, or chronic conditions. This role requires not only clinical expertise but also strong interpersonal skills to understand patient needs and motivate them throughout their recovery journey.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Valid State License in Physical Therapy for Virginia.
• A minimum of two years of hands-on experience in physical therapy practice.
• Effective verbal and written communication abilities, essential for collaboration with patients and interdisciplinary teams.
• Proficient in utilizing technology for documentation and patient care management.
• A positive attitude with a commitment to providing excellent care in a flexible and agile work environment.

Preferred Experience

• Experience in home health care or working with diverse patient populations.

Compensation and Benefits

This position offers a competitive annual salary ranging from $90,000 to $110,000, commensurate with experience.
