Pay: 22.28/hr-33.42/hr

Make a real difference in patients' lives by investigating, assessing, and planning meaningful interventions that empower patients to overcome social, emotional, economic, and environmental challenges during their neurological recovery journey.

 

Responsibilities:

Connect with patients in their own homes throughout the tri-county area (primarily within 10 miles of Rochester, with commutes up to 45 minutes). Build trust and understanding through thoughtful interviews, comprehensive assessments, and thorough documentation of each patient's unique situation and progress.

 

Collaborate with team managers, statutory and voluntary agencies, and patients' families to create cohesive, compliant treatment plans that truly address each patient's needs and goals.

 

Empower clients and families by connecting them with vital community resources and navigating the referral process to ensure they receive the support they deserve.

 

Build and nurture strong partnerships with community health, welfare, and social agencies. Bring creative problem-solving to every challenge, finding innovative solutions that help clients achieve their goals.

 

Champion seamless transitions by actively managing concurrent reviews, discharge planning, and post-discharge needs—ensuring continuity of care and setting patients up for success beyond the clinical setting.

 

Qualifications

Education: 

• Bachelor's Degree in Social Work - Required 
• Must be OK with commuting to patient homes
