Patient Coordinator

Your future role at a glance Location: Indianapolis, IN Facility: St. Vincent PACE Department: Home Care Schedule: Days | Full-time How you’ll make an impact in this role Scheduling & Referrals: Coordinate external medical appointments, referrals, and schedule changes for assigned patients. EHR Documentation: Log all appointments, special instructions, and patient details accurately into the electronic medical system. Provider Contact Management: Maintain and regularly update the external provider contact directory. Protocol & Compliance: Follow site-specific workflows and keep documentation up to date to ensure compliance. What minimum requirements you’ll need Licensure / Certification / Registration: Education: High School diploma equivalency OR 1 year of applicable cumulative job specific experience required. Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
