Clinical Pharmacist - Overnight ICU - 7 on/7 off

Job Summary Provides leadership in a specialized healthcare team and the patient to assure optimal cost-effective drug therapy. This includes developing specific pharmacotherapeutic goals, recommending a regimen and developing a monitoring plan to meet the individualized drug therapy needs of the patient. The clinical pharmacist is responsible for leading efforts in the prevention and reporting of medication errors and adverse drug reactions according to policy and works to continuously improve process and practice in the institution to enhance safety of medication therapy. Position Highlights 7 on/7 off schedule. Hours are 7:00pm - 7:00am. This position will primarily support caring for patients in various ICUs during the overnight hours with some coverage in the emergency department. In addition, this Clinical Pharmacist will have responsibility for consult coverage, clinical questions, and code response for admitted patients in the adult hospital. Opportunities for engagement with other shifts, projects, precepting and committees are available based on individual's interests. We are looking for motivated individuals to help expand clinical scope and practice. Preferred Qualifications PGY2 in ICU or emergency medicine or equivalent experience. PGY1 plus experience in critical care setting. Key Responsibilities Reviews drug orders and/or prescriptions to ensure the safe, appropriate and cost-effective use of medications. Prepares and dispenses medications, provides drug information and manages medication therapy in accordance with federal, state and local regulations and hospital policy. Provides outstanding patient care by optimizing medication preparation and resolving medication-related problems. Maintains the quality, safety, and consistency of clinical pharmacy services in assigned areas. Supervises the activities of assigned support personnel and verifies the accuracy and appropriateness of all medications prepared by support personnel prior to dispensing to patients or dispensing to healthcare providers for administration to patients. Technical Capabilities Mentoring & Coaching (Intermediate) : Demonstrates good training and mentoring practices on a regular basis. Acts as a mentor to a person in need of support and direction. Makes a concerted effort to seek problem definition so it is clearly understood, clarifying and refining goals, as needed. Encourages two‑way dialogue and explores alternative solutions and approaches. Medication Management (Advanced) : Demonstrates advanced expertise in medication management in challenging and complex situations. Determines clinical appropriateness of each medication, dose, indication, contraindications, potential adverse effects and potential problems. Monitors therapeutic duplication or unnecessary medications, interprets laboratory results and provides drug information. Pharmaceutical Drug Preparation & Dispensing (Novice) : Possesses sufficient fundamental proficiency to review, prepare and dispense prescribed medications, including aseptic technique for sterile preparations. Pharmaceutical Quality Assurance & Compliance (Expert) : Demonstrates expert knowledge of rules and regulations and leads process improvement initiatives to ensure continuous quality improvement and regulatory compliance. Pharmaceutical Computer Literacy (Novice) : Proficient in pharmaceutical computer applications to support medication use processes. Education and Experience Education: Bachelor's degree required. Experience: 1 year relevant experience in a clinical pharmacy setting. Certifications: Licensed pharmacist (Tennessee).
