Work Environment

This role will involve onsite, hands-on patient care in various home settings across Santa Rosa. Key Responsibilities

• Provide evidence-based orthopedic therapy directly in patients' homes, ensuring a safe and effective treatment experience.
• Work closely with in-house medical professionals, including MDs, Occupational Therapists, and other Physical Therapists, to develop integrated care plans tailored to individual patient needs.
• Conduct comprehensive assessments of patient safety, mobility, and functional capabilities within the home environment, adapting therapy interventions as necessary to optimize outcomes.
• Educate patients and their families on exercises, body mechanics, adaptive strategies, and home modifications to enhance the recovery process.
• Utilize secure electronic medical record (EMR) tools to meticulously document patient sessions, progress, and adjust treatment plans based on patient response.
• Participate in interdisciplinary case reviews and ongoing clinical education to stay up-to-date with best practices and improvements in patient care.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic surgeries, injuries, and those dealing with chronic musculoskeletal issues. You will work with a diverse range of individuals who may require tailored modifications in their rehabilitation approach depending on their home environment and personal challenges.

Qualifications

• Current license to practice as a Physical Therapist in the State of California (or eligibility to obtain one).
• A Bachelor's Degree in Physical Therapy or equivalent qualifications recognized by the state.
• Demonstrated interpersonal and organizational skills essential for fostering strong patient relationships and managing a caseload effectively.
• Independence and flexibility in managing homecare responsibilities, along with a proactive approach to work.

Preferred Experience

• At least 2 years of experience in a homecare or outpatient orthopedic setting is preferred; however, confident new graduates who possess a strong willingness to learn will also be considered.
• A passion for providing personalized, patient-centered care, understanding the nuances of delivering services in a home setting.
