The practice delivers comprehensive care across outpatient clinics, inpatient settings, skilled nursing facilities, and long-term care hospitals. The practice is strongly integrated with Prime Health Care through the EPIC electronic health record, offering streamlined access to patient charts, lab results, and appointment reminders. It is also an active participant in clinical integration initiatives with Ascension Care Partners, supporting high-quality, coordinated care. Role Description This is a full-time, on-site Family Nurse Practitioner role based in Chicago, IL. The Family Nurse Practitioner will provide direct primary care services across the continuum, including health assessments, diagnosis, treatment, and follow-up for patients in outpatient, inpatient, and post-acute care settings. Daily responsibilities include managing chronic and acute conditions, ordering and interpreting diagnostic tests, prescribing medications, and coordinating care with physicians and other healthcare professionals. The role involves using EPIC to document encounters, review lab results, and maintain accurate patient records, while ensuring compliance with clinical guidelines and practice policies. The Family Nurse Practitioner will also participate in care planning, patient education, and quality improvement activities that align with clinical integration projects and value-based care initiatives. Qualifications Demonstrated clinical expertise in Nursing and advanced practice roles, including experience as a Nurse Practitioner . Strong background in Primary Care , with proficiency in managing chronic disease, preventive care, and family-focused health services. Solid foundation in Medicine relevant to outpatient and inpatient primary care, including diagnostic reasoning and evidence-based treatment. Current Nurse Practitioner license in Illinois and national certification as a Family Nurse Practitioner (FNP) or equivalent. Graduate degree from an accredited Nurse Practitioner program (MSN or DNP) and active BLS certification; ACLS preferred. Proficiency with electronic health records, ideally EPIC, and strong documentation and organizational skills. Excellent communication, collaboration, and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
