Key Responsibilities\n

• Deliver clinical services to assigned clients experiencing symptoms of major mental illness or severe emotional disorders through telehealth and community outreach efforts.

• Establish a trusting relationship with clients by demonstrating empathy, proactive listening, and respect for each individual’s cultural and personal background.

• Collaborate with multidisciplinary teams to advance common patient care goals, ensuring effective communication and coordination of care.

• Conduct comprehensive assessments and screenings, utilizing evidence-based practices to identify client needs and create tailored treatment plans.

• Implement integrated care plans, linking clients with necessary services and facilitating information exchange among clients, families, and service providers.

• Provide focused intervention services, including brief treatment and recovery services, ensuring that each client’s unique situation is addressed.

• Maintain high standards of safety by identifying potential hazards and employing appropriate protocols to secure a safe environment for all clients and team members.

• Crisis intervention, as necessary, to ensure immediate support and risk management for clients in distress.

• Carry a manageable caseload, providing individual, group, and family therapy sessions while actively monitoring treatment progress.

• Ensure timely completion of clinical documentation, reports, and data submissions following agency protocols.

• Participate in quality improvement committees to enhance agency service delivery and patient outcomes.

Patient Population / Client Focus\n

You will primarily support clients who are experiencing a range of mental health issues, including depression, anxiety, and other severe emotional disorders. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current and valid LCSW license.

Compensation and Benefits\n

The salary range for this position is competitive, from $70,000 to $85,000 annually, commensurate with experience and qualifications.
