Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to identify emotional, social, and environmental factors influencing patient recovery, utilizing interviews, observations, and medical record reviews.

• Provide compassionate counseling and support to patients and their families, addressing adjustment issues related to terminal illness, emotional well-being, and end-of-life decisions.

• Collaborate closely with physicians, nurses, and other therapists to develop and adapt individualized treatment plans that reflect the psychosocial needs and goals of each patient.

• Assist patients in navigating community resources, including support groups, occupational therapy, and financial assistance programs to help them manage their conditions effectively.

• Facilitate discharge planning, ensuring a smooth transition for patients moving back home or to other care settings, while also coordinating with family members to prepare for ongoing support.

• Participate in multidisciplinary team meetings and contribute insights regarding psychosocial interventions that promote patient outcomes and enhance the quality of care.

Qualifications\n
