At Altais, we support one another, adapt quickly, and work with integrity as we build a better experience for physicians and their patients.Our Nurse Practitioner will perform comprehensive health assessments which include but not limited to the following:Obtain history and perform physical examinationComplete review of medicines and related instructionsReview home safety and psychosocial needsFulfill documentation needs for STAR and HCC measuresObtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissionsInstruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergenciesMaintain patient safety and confidentialityUniversal precautions utilized in all patient carePrescribe/order procedures, treatments and medications within scope of practiceDevelop high level of trust and intimacy with patients and familiesThe Skills, Experience & Education You BringCA Licensed Nurse Practitioner1+ Years of Experience with Primary CareThe Base Salary for this position is $135,000 - $155,000/yearSign on Bonus: $25,000In addition, we provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.Excellent medical, vision, and dental coverage401k savings plan with a company matchFlexible time off and 9 Paid HolidayAltais values the contribution each Team Member brings to our organization.
