Position Overview

We are seeking a compassionate and experienced Registered Nurse (RN) to join an outpatient primary care team in the Bronx. The RN will provide direct clinical support to patients and providers, assist with care coordination, patient education, clinical assessments, and routine nursing responsibilities in a community health center environment.

Responsibilities

Perform nursing assessments and obtain patient medical histories and vital signs. Provide direct nursing care and clinical support to patients in an outpatient setting. Assist providers with examinations, procedures, and treatments. Administer medications, immunizations, and treatments as ordered. Provide patient education regarding medications, treatment plans, preventive care, and disease management. Coordinate follow-up care, referrals, testing, and other patient services. Monitor and document patient conditions and clinical information accurately in the electronic medical record. Communicate effectively with physicians, NPs, PAs, medical assistants, and other members of the care team. Support preventive health initiatives and chronic disease management. Maintain compliance with organizational policies, clinical standards, and regulatory requirements. Provide culturally sensitive and compassionate care to a diverse patient population.

Qualifications

Active New York State RN license. Graduate of an accredited nursing program. Current BLS certification. Prior outpatient, primary care, community health, or FQHC experience preferred. Strong clinical, communication, organizational, and documentation skills. Ability to work effectively in a multidisciplinary team. Bilingual skills are a plus.
