Key Responsibilities\n

• Participate in discharge planning and aftercare initiatives, working with clients to establish a sustainable support framework post-treatment.

• Provide therapeutic meal support, helping clients develop positive relationships with food while modeling healthy eating behaviors.

• Deliver empathetic and affirming care tailored to each client’s unique circumstances, considering their intersecting identities and display of cultural humility.

• Maintain accurate clinical records in compliance with state regulations and organizational standards, using our electronic health record (EHR) system to document client interactions and treatment plans.

Patient Population / Client Focus\n

As an LCSW at Adelante Life Solutions, Inc., you will primarily work with a diverse client base, with a special focus on older adults who may be facing complex mental health challenges, including anxiety, depression, and adjustment disorders. Compensation and Benefits\n
