Key Responsibilities\n

• Act as a liaison between families, clients, and outpatient providers, ensuring effective communication and coordination of services.

• Engage with insurance companies for pre-certification processes and participate in utilization management to secure necessary services for clients.
\n
• Facilitate discharge planning and aftercare services for clients transitioning from higher levels of care to ensure continuity in their mental health journey.
\n
• Provide therapeutic meal support, exemplifying and encouraging healthy relationships with food for clients, particularly those struggling with eating disorders or other dietary challenges.
\n
• Deliver safe, empathetic, and affirming care tailored to the unique needs of each client, taking into account their various intersecting identities and experiences.

Patient Population / Client Focus\n

Our primary focus is on older adults, many of whom deal with complex emotional and psychological issues. This role requires an understanding of the unique challenges faced by this demographic, such as navigating life transitions, managing chronic health conditions, and addressing grief or loss. The LCSW will provide individualized support to empower these clients to achieve their highest potential for well-being and personal growth.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active LCSW licensure in the State of Virginia is mandatory.

• Demonstrated experience working with older adult populations, providing tailored behavioral health services.

Compensation and Benefits\n
