Clinical Case Manager: Mental Health & SUD Specialist

PRC Baker Places seeks a Clinical Case Manager to deliver case management and clinical services for adults with mental health and substance use disorders in the Baker Places program. You will coordinate care with the housing and entitlements team and ensure high standard of client care. You will perform psychosocial assessments, develop treatment plans, and provide crisis response and on-call coverage as needed.
