Responsibilities

• Conduct comprehensive PT evaluations for residents to determine mobility, balance, strength, and functional goals.
• Develop, implement, and monitor individualized treatment plans to improve gait, functional independence, and activity tolerance.
• Deliver direct therapy sessions, document progress, and adjust treatment plans based on patient response.
• Utilize therapeutic exercises, manual techniques, and modalities as appropriate; ensure proper use of assistive devices and safety during activities.
• Collaborate with nursing, rehab, and social work teams to coordinate care, discharge planning, and goal attainment.
• Maintain accurate records in accordance with facility policies and regulatory requirements; ensure infection control and patient privacy throughout care.

Requirements

• Active Physical Therapist license to practice in Maine
• Ability to fulfill a 13-week travel assignment
• Strong communication and teamwork skills, with the interdisciplinary care team
• Ability to work in a skilled nursing facility setting and adhere to clinical safety standards
• Commitment to resident-centered care and thorough documentation
