The successful candidate will not only provide expert counseling services, especially to patients dealing with co-occurring disorders, but will also play a crucial role in promoting long-term recovery for individuals grappling with addiction issues.

Key Responsibilities\n

In your role as an LCSW, your primary responsibilities will include:

• Conducting thorough assessments to identify the counseling needs of clients and establishing a treatment plan tailored to their individual circumstances.

• Utilizing DSM (Diagnostic and Statistical Manual) criteria for substance abuse diagnosis and determining the appropriate level of care in accordance with ASAM (American Society of Addiction Medicine) standards.

• Preparing the substance abuse counseling components of client treatment plans, ensuring a holistic approach to each individual's recovery journey.
\n
• Developing detailed discharge and continuum of care plans that facilitate smooth transitions for clients to appropriate levels of care.
\n
• Documenting all counseling interactions, assessments, and referrals clearly and comprehensively in each client's clinical record.
\n
• Maintaining an organized caseload, encompassing all necessary case management activities and clinical charting as mandated.
\n
• Coordinating support services for clients and their families, documenting these interactions meticulously in the clinical records.
\n
• Engaging with referral sources such as legal entities, schools, and employers to ensure a comprehensive support network is established for each client.

• Facilitating crisis interventions, employing a multi-disciplinary team approach to address urgent issues effectively.

Patient Population / Client Focus\n

The primary focus of this position will be on individuals dealing with substance use disorders and co-occurring mental health issues. You will be responsible for implementing therapeutic interventions that not only address addiction but also help clients navigate their overall well-being. You will work closely with a diverse population, allowing you to leverage your cultural competency and therapeutic skills in meaningful ways.

Qualifications\n

The ideal candidate for this role should have:

• A Bachelor’s degree in Social Work or a related field.

• A current, valid LCSW licensure.
