Licensed Psychiatric Case Manager
Lehigh Valley Hospital
Lehigh Valley Health Network seeks a Licensed Psychiatric Case Manager to coordinate compassionate, patient-centered mental health care across our regional system. In this role, you will complete psychosocial assessments, develop and monitor individualized care plans, and connect patients and families with essential community resources. You’ll collaborate closely with psychiatrists, nurses, therapists, and social services to support safe transitions of care. LVHN offers a collaborative, learning-focused culture, strong clinical partnerships, and opportunities to advance your career while improving community mental health in eastern Pennsylvania.
