Work Environment

This role involves working onsite, primarily within a home care setting. You will have the chance to build meaningful relationships with patients as you visit them in their homes, allowing for a comfortable therapeutic environment that promotes healing. Working alongside a multidisciplinary team, you will engage with other healthcare professionals to develop strategies for integrated patient care.

Key Responsibilities

• Conduct comprehensive evaluations of each patient's condition upon intake to establish an accurate diagnosis and treatment plan tailored to their specific needs and recovery goals.
• Utilize a variety of evidence-based treatment methodologies, including manual therapy techniques, therapeutic exercise protocols, and modalities such as ultrasound therapy, to facilitate optimal patient outcomes.
• Monitor and document patient progress meticulously, ensuring that all treatment sessions and progress notes are recorded in accordance with clinic standards and compliance guidelines.
• Adjust treatment plans as necessary based on ongoing assessments of patient progress, and maintain open lines of communication with patients to provide continuous support and motivation.
• Educate patients on their conditions and empower them with customized home exercise programs designed to enhance their recovery and maintain their health outside of therapy sessions.
• Collaborate closely with physicians and other healthcare providers to offer a comprehensive approach to patient care, ensuring each patient's treatment plan addresses the whole body.
• Establish rapport with patients and their families, actively engaging them in the rehabilitation process and fostering an environment of encouragement and trust.

Patient Population / Client Focus

Your primary focus will be on patients dealing with orthopedic conditions, post-surgical recoveries, and chronic pain management. You will provide specialized care that respects each patient's preferences and promotes active participation in their healing journey, which is crucial for effective rehabilitation, particularly in a home health setting.

Qualifications

• Bachelor's degree in Physical Therapy or related field from an accredited institution.
• Active California state licensure as a Physical Therapist.
• In-depth understanding of human physiology and anatomy as it pertains to physical therapy practices.
• Demonstrated ability to create customized treatment plans that reflect best practices in orthopedic rehabilitation.
• Strong communication skills for effective interaction with patients, families, and healthcare team members.
• Ability to work independently and collaboratively as part of a dedicated team focused on patient care.

Preferred Experience

• A minimum of two years of direct experience in outpatient orthopedic settings is highly desirable.
• Experience with sports rehabilitation, post-surgical recovery plans, or chronic pain management is preferred.
• Familiarity with electronic health record systems is advantageous for documenting patient progress and treatment plans efficiently.

Compensation and Benefits

We offer a competitive annual salary ranging from $110,000 to $125,000, commensurate with experience, along with a robust benefits package that includes health insurance, paid time off, and a 401(k) plan with employer matching contributions.
