Dedicated Care Team to assist with care gaps, patient outreach, care management, post-ER and admission monitoring, post-discharge follow-up care, and RX support.Closed ICU, no procedures with 7 on / 7 off schedule. $50,000 sign on bonus. oin an expanding Hospitalist team - currently 2 Hospitalists (growing to 4), 2 daytime APPs, and 4 nocturnal APPs. Internal Medicine division currently comprised of 20+ Internists and 4 Clinic-Based APPs. Located in Downtown Des Moines. Dedicated Nocturnal APPs take first call from 4:30pm-6am, 7 days a week.
