Job Description

Our organization is looking for a physician trained in Internal Medicine or Family Medicine to become part of our expanding Senior Health team. The selected individual will play a vital role in delivering home-based primary care within the community.

This role will contribute to the advancement of our Complex Care Institute, which aims to 'meet individuals where they are and advocate for what matters most.' This dedicated team currently serves over 500 patients, focusing on providing comprehensive, interdisciplinary primary care and support to our most vulnerable and complex patients in a cost-effective and well-coordinated manner.

The successful candidate will join our Medical Group, a multi-specialty practice that includes nearly 700 providers affiliated with our healthcare system. With two hospitals and more than 20 health centers, we are recognized as one of the largest integrated healthcare delivery systems in the region.

Opportunity Highlights

Full-time position with a robust benefits package and competitive salary, including a salary guarantee.
Our Senior Health Center is nationally recognized for its innovative care model for seniors, promoting an integrated approach that improves care quality and service accessibility.
Work schedule is Monday through Friday during standard business hours.
Minimal on-call duties, with a weekend call rotation of 1:4.
Travel radius is limited to 30 minutes from the hospital, with mileage reimbursement provided.
Collaborative work environment with advanced practice providers, social workers, and pharmacists.
Opportunity for an ACGME accredited Geriatric Fellowship for interested candidates.
Active participation in the education of medical students, residents, and fellows.
We offer an academic model of care within a community teaching hospital setting.
Utilization of Epic Systems for electronic medical records.
Implementation of Ambient Listening technology.

Candidate Requirements

Board Certified or Board Eligible in Internal Medicine or Family Medicine.
Possession of an unrestricted Ohio medical license or the willingness to obtain one.
A strong commitment to delivering quality care in a patient-centered, team-oriented environment.
