Intake CoordinatorPUEBLO, CO 81008Intake CoordinatorJob Summary: The Intake Coordinator's primary duty is to facilitate the intake referral process and coordinate care with the interdisciplinary team and the referral source. This position requires skill in planning, organizing and coordinating the delivery of patient care at the point of the patient's referral. The Clinical Intake Coordinator makes direct contact with referring physicians, hospital liaisons and patient/families as it relates the delivery of patient care at the point of a patient's referral.Gathers pertinent information about the referred patient to include demographic, clinical and payer source information and completes written referral formObtains the official written physician's order for start of careProvides the Billing Department with complete and accurate insurance informationCommunicates with the DON/ADON/Admissions Nurse etc. any information received from the physician, patient or employees about the patient's care, needs or accepting the referralEnters referral information into EMR; including scanning intake orders and clinical notes, keying patient demographics, and caregiver contact informationCreates and maintains all patient hard files in office and ensures all documents that are required are received and filed and are readily availableCoordinates with DON/ADON/Admissions Nurse an evaluation visit/admission visit with the patient and family as requested and appropriateCollaborates with referral sources to facilitate transition to hospiceOrders and arranges for DME and supply deliveryKeeps an intake log of all referrals and collects and maintains statistical data on all non-admissions and admissions and reports information monthly, and as requested.Ensure compliance with all Conditions of Participation and Agency Admission policy and proceduresAnswers telephone lines promptly and efficiently. Responds to message content appropriately; disseminates information to the appropriate personsSurveys patients/families to determine level of satisfaction with agency services. Documents findings and notifies appropriate supervisor of any dissatisfied customersMaintain and track office supply orders, stock printer paper with paper nightly to ensure after hour faxes and documents come through timely.Gets the office ready for the business day to include checking the fax machine/printer for any new business.
