Intake Specialist

Conduct intakes for new clients and provide orientation to the program/shelter and DHS rules and regulations.

Review documents to ensure all new client intakes are completed within 24 hours as required.

Maintain required documentation in case records in written and computer databases (CARES) and update client's records in accordance with policy.

Develop an individualized and comprehensive intake/assessment to fully identify client needs.

Collect client identification and other documents necessary during initial intake.

Assist new clients in retrieving documents and/or property from former shelter providers as necessary.

Collaborate with other shelter providers to request and obtain missing client information required (i.e. medical, psych, entitlement and/or housing information, etc...).

Collaborate with Health Services to register clients with the clinic and initial evaluation appointments are issued.

Deconstruct closed case files and discharged client files.

Follow up with and submit referral packages to external shelter providers when clients are transferred.

Input new clients on weekly data tracking spreadsheets.

Escort clients to appointments as needed.

Collect and supply data and information necessary for monthly reports to the Administrator.

Establish and maintain a network of referral resources.

Participate in regular supervision, team meetings and staff training.

Perform all other duties as assigned by the Supervisor or designee.

Promote good community relations through participation in community affairs and community outreach programs as assigned.

Participate in Job Training and Education Programs, Meetings and Committees.

Attend mandatory in-service annually.

Under the direction of the Director of Operations, the Intake Specialist is to ensure all new clients are intake and provides orientation/coordination to the program/shelter services. This position will ensure the delivery of services to all clients on a 24-hour basis as per the Department of Homeless Services (DHS) contract and VIP Community Services. This position will work with individuals or families with children dealing with various issues, such as homelessness, economic hardships, mental health issues and/or seeking asylum.

Requirements

High School/General Equivalency Diploma and minimum 1-year operational experience with homeless and/or substance use populations.

Crisis Intervention and client advocacy experience a plus.

Knowledge of the homeless population, substance use disorders and/or mental health a plus.

Excellent interpersonal, written and oral communication skills.

Ability to work a flexible schedule and occasionally work shifts associated with a department that operates 24 hours per day, 365 days per year.

Fire Safety Coordinator Certificate or received within 30 days of hire.

NYC Food Handler's Certificate or received within 30 days of hire.

Basic First Aid received within 30 days of hire.

CPR received within 2 months of hire.

Ability to interact effectively with all populations served.

Fluent in written and spoken English language.

Bilingual (English/Spanish) Preferred
