Overview
Licensed Practical Nurse (LPN) – Long-Term Care / Skilled Nursing Facility – Utica, NY. This travel - temporary assignment through Healthforce Healthcare LLC starts 09/14/2026 for contract duration of 13 weeks. Weekly pay ranges from $1,442 to $1,528 per week. The assignment offers 37.5 hours per week with a night shift from 10:30 pm to 6:30 am. Work is in a 40-bed long-term care unit providing rehabilitation and memory care services. The role requires active NY LPN licensure, BLS certification, and LTC experience; EMR experience with PointClickCare is preferred. RTO must be included at submission. Local candidates within 75 miles may be considered at local rate.

Responsibilities
- Administer prescribed medications and treatments per nursing orders; monitor for adverse effects and document outcomes.
- Provide direct patient care to residents with diagnoses including MS, orthopedic concerns, and dementia/memory care; assist with activities of daily living.
- Monitor patient status, communicate changes to the care team, and participate in care planning with RNs and interdisciplinary staff.
- Document care activities in PointClickCare EMR; maintain accurate MAR, flowsheets, and shift handoffs.
- Respond to call lights promptly; ensure patient safety, mobility, and fall prevention; assist with transfers and activities as needed.
- Float within the same building per unit guidelines; adhere to facility policies and infection control practices.

Requirements
- Active Licensed Practical Nurse (LPN) license in New York
- Current BLS certification
- 1+ year LTC facility experience; travel experience documented on resume
- Proficiency with PointClickCare or willingness to learn EMR software
- NURSYS license verification showing an ACTIVE NY LPN license and all active licenses
- RTO (time-off) must be included at submission
- Driver’s license with address that matches demographics
- Ability to work the listed shift and to float within the same building as needed
