Job Description
Title: LPN - Long Term Care

Schedule:
- Shift: 6:30 AM - 2:30 PM
- Days: Every other weekend required

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 37.5 per week

Requirements:
- Active LPN license in New York
- 1+ years of experience in a long-term care facility
- Current BLS certification
- Experience with PointClick Care EMR
- Skills checklist matching specialty
- Valid driver's license with matching address

Roles & Responsibilities:
- Provide nursing care to residents in accordance with their care plans.
- Assist with daily living activities including personal hygiene, mobility, and nutrition.
- Monitor residents' health and document changes or concerns.
- Collaborate with interdisciplinary team members to ensure optimal care for residents.
- Administer medications as prescribed and adhere to safety protocols.
- Maintain clean and safe environment for residents.

Compliance / Notes:
- RTO must be included at time of submission.
- Scrub color: Navy blue.
- Floating required within the same building.
