Key Responsibilities\n

• Work closely with primary care practitioners to identify, assess, and manage mental health concerns, thereby enhancing the efficiency of the healthcare team.

• Conduct thorough initial patient intakes focusing on diagnostic and functional evaluations, which will inform the development of therapy goals and treatments.

• Provide emergency mental health interventions as necessary, delivering timely care to those in crisis.

• Advise primary care practitioners on appropriate care pathways for patients, including recommendations for specialized mental health services and referrals.

• Regularly monitor patient progress through follow-up evaluations, adjusting treatment plans to ensure effectiveness.

• Collaborate with patients to create personalized relapse prevention plans that address their unique needs.

• Implement screening protocols to identify at-risk patients and actively engage in developing preventive healthcare programs.

• Document patient histories and progress accurately and comprehensively in accordance with agency policies.

• Maintain ongoing communication with other healthcare providers to ensure cohesive treatment planning.
