Key Responsibilities\n

• Conduct comprehensive patient intakes focusing on diagnostic assessments, functional evaluations, and co-develop therapy goals in collaboration with primary care teams.

• Deliver immediate and focused mental health interventions to patients requiring emergency treatment, ensuring swift and appropriate support.

• Advise primary care providers on patient suitability for primary care interventions versus specialized mental health services, guiding referrals as necessary.

• Implement regular follow-up evaluations to monitor treatment effectiveness, adapting care plans based on individual patient progress.

• Assist patients in developing relapse prevention strategies and provide ongoing support throughout treatment.

• Coordinate mental health care services internally within the primary care practice and externally with other providers, securing patient consent where needed.

• Maintain accurate documentation of patient evaluations, progress notes, and treatment plans, adhering to the standards of Agency and Federally Qualified Health Center (FQHC) compliance.

• Foster strong communication channels with primary care providers to ensure a cohesive understanding of patient needs and treatment plans.

• Educate patients on self-management skills and promote lifestyle changes conducive to both their mental and physical health.

Patient Population / Client Focus\n

Qualifications\n

• A Master's Degree in Clinical Counseling is required.

• A valid Licensed Professional Counselor (LPC) designation.

Compensation and Benefits\n

We offer an attractive salary range of USD 90,000 – 105,000 annually, commensurate with experience and qualifications.
