Job Title: Clinical Pharmacist

Location: Unity Hospital Hospitalist

Department: Inpatient Pharmacy

Hours Per Week: Full-time

Schedule: 7 on, 7 off, Mon-Sun. Hours are 1:30p -12a.

Summary:Works collaboratively with all health care providers to optimize patient care. Responsible for overall dispensing duties, preparation of IV admixtures, and maintaining up-to-date knowledge in the field to function as a drug consultant to all members of the medical profession. Coordinates activities of the Pharmacy Department in accordance with the established policies and procedures of Unity Health System Pharmacy. Assists the department in providing pharmaceutical services, including advising providers on drug use and control within the assigned hospital.Key Responsibilities:

• Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes, including advising providers on issues concerning drug therapy, the inherent toxicity of drugs and side effects, as well as assisting in the prescription of appropriate doses.

• Coordinate the activities of assigned pharmacy technicians engaged in the preparation, labeling and delivery of medications and other pharmaceutical supplies in accordance with providers’ prescriptions: ensure compliance with federal, state and local laws and regulations about the dispensing of drugs and controlled substances and the maintenance of required records.

• Maintain liaison relationship with medical and nursing staff; provide timely information about pharmaceutical supplies, drug usage and compatibilities, state and federal regulations regarding drug controls, and Joint Commission standards.

• Prepares TPN, chemotherapy, and other IV products.

• Conducts and evaluates medication histories, assesses compliance and suggests modifications to achieve desired outcomes.

• Mentor/Supervise pharmacists in training, including but not limited to pharmacy interns; activities should include both practical and didactic.

• Utilizes independent judgment to decide what to do to assemble facts, determine variations from standard procedures, or plan other actions to ensure the operation of the pharmacy. Suggests improvement in work methods.

• Individualizes medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption.Adheres to the Unity Health System CORE values.

• Anticipates and contributes to management and professional committees and special meetings at the hospital, community, etc. Keeps informed on developments and trends inside and outside the hospital which relate to assigned functions. Participates in hospital in-service programs when required. Performs other duties as assigned by Pharmacy Management efficiently and cooperatively.

Required Qualifications:

• Bachelor of Science in Pharmacy. Or Doctorate of Pharmacy Degree.Three years of hospital experience preferred or a demonstrated ability to achieve positive patient outcomes in a similar environment

Required Licensure / Certification Skills:

• Current New York State License.

Light - Clinical - Light clinical roles involve frequent standing and walking, with occasional lifting of light objects (≤20 lbs.) and routine reaching, grasping, and fine manipulation for tasks such as patient care, laboratory work, or specimen processing. Staff rely on strong visual and auditory acuity to accurately observe specimens, read instrumentation, monitor patients, respond to alarms, and communicate effectively with care teams. Cognitive demands include multitasking, prioritizing clinical and laboratory workflows, and coordinating with interdisciplinary teams. Emotional resilience is necessary due to frequent interruptions, shifting priorities, and the need for precise and compassionate interaction with patients and colleagues. Environmental exposures may include infectious materials, biological specimens, cleaning agents, sharps, chemical reagents, and wet surfaces. PPE use, TB testing, and schedule flexibility are commonly required.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
