We are seeking a talented Social Worker/Case Manager to join our team About the Role As a Social Worker/Case Manager, you will play a vital role in our interdisciplinary team, providing comprehensive assessments, counseling, and case management services to our residents. Your expertise in psychosocial needs, adjustment to illness, and community re-entry barriers will enable you to develop and implement individualized care plans, ensuring our residents receive the highest level of care and support. Responsibilities Conduct comprehensive admission intake/psychosocial assessments and trauma-informed assessments Develop and implement individualized care plans Provide counseling, individual, group, and family Facilitate comprehensive care plans with interdisciplinary teams, resident neighbors, families, and designated representatives Explore discharge planning/community options, including TBI waiver, NHTD waiver, OPWDD waivers, and traditional discharge services Maintain open lines of communication with designated representatives, legal guardians, and other healthcare professionals Participate in Performance Improvement activities and enhance professional growth through educational in-services and community involvement Master Level Supervision available, with flexible scheduling options Requirements Bachelor of Social Work (BSW) degree or bachelor's degree in a related field with one year experience in social work or case management Professional demeanor and excellent communication skills Experience with office technologies, computers, word processing, and software programs Strong organizational skills and ability to prioritize tasks Ability to read, write, speak, and understand the English language Possess patience, tact, cheerful disposition, and enthusiasm Must be able to make independent decisions and work harmoniously with other personnel Ability to plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures Compensation Commensurate rate with experience Benefits Competitive salaries Medical, dental, and vision coverage available 401K with employer contribution Generous Paid Time Off Daily Pay Raffle prizes given weekly/monthly/annually Flexible scheduling Increased pay rates Referral bonuses Why work at Northeast Center for Rehabilitation and Brain Injury?
