Job Title: Clinical Nurse Profession: RN Specialty: Stepdown Duration: 9 months Shift: Night Shift Hours per Shift: 11.5 hours Experience: - Minimum of two (2) years of cardiology, telemetry, and VAD experience - Minimum of two (2) years RN experience License: - Active RN License Certifications: - BLS Must-Have: - Critical thinking skills - Aligned with evidence-based practice Description: The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Utilizes the nursing process based on theory, research, and evidence-based practice. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team. Manages assignments within the care delivery model and demonstrates effective allocation of material resources. Role models the tenets of Relationship Centered Care through various patient engagement actions. Considers the patient's values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with the patient and family. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and family members with concerns. Demonstrates caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions. Conducts purposeful hourly rounding on assigned patients and addresses essential patient care needs. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on institutional evidence and procedures. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects and practice activities. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high-risk behaviors, ensuring a safe environment for patients.
