Job Description

Schedule:

• Monday Friday, 8:00 AM 5:00 PM

Role Highlights:

• Manage a caseload of approximately 30 points per week (average of 35 patient visits daily)
• Provide care directly in patient homes within an assigned area, with mileage reimbursement for travel
• Utilize Home Care Home Base (HCHB) for all documentation
• Conduct detailed OASIS assessments and formulate personalized plans of care

Key Responsibilities:

• Complete comprehensive OASIS assessments to guide individualized patient care
• Deliver skilled nursing services, including wound care, IV therapy, medication administration, and management of chronic illnesses
• Monitor and assess patient progress, communicating changes to the interdisciplinary team and physicians
• Educate patients and families regarding conditions, care plans, and preventative health strategies
• Maintain thorough, timely documentation utilizing HCHB
• Collaborate with therapy staff, social workers, and home health aides to ensure seamless care
• Attend case conferences, staff meetings, and participate in ongoing training
• Uphold regulatory and organizational best practices for safe and effective care delivery

What Were Looking For:

• Current, active RN license in good standing (state of assignment)
• At least one year of recent field home health nursing experience
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments
• Dependable transportation, a valid drivers license, and current auto insurance
