Position Overview The Family Nurse Practitioner will provide comprehensive primary care services to patients across the lifespan, collaborating closely with our physicians and clinical staff to deliver exceptional outpatient care. We are looking for a provider who values continuity of care, preventive medicine, and building meaningful relationships with patients in a community-based setting. Responsibilities Conduct comprehensive patient assessments and physical examinations Diagnose and manage acute and chronic medical conditions Develop and implement treatment and care plans Order, interpret, and review laboratory and diagnostic tests Prescribe medications and treatments in accordance with New York State regulations Provide preventive care, wellness visits, and health education Manage chronic diseases such as diabetes, hypertension, and hyperlipidemia Maintain accurate and timely electronic medical records Collaborate with physicians and staff to coordinate patient care Participate in quality improvement initiatives and patient outreach efforts QualificationsRequired Licensed Family Nurse Practitioner in New York State Current DEA registration (or eligibility to obtain) Master's or Doctorate degree in Nursing from an accredited program Board Certification as a Family Nurse Practitioner Strong clinical and communication skills Commitment to delivering compassionate, patient-centered care Preferred Experience in primary care, family medicine, internal medicine, or outpatient practice Spanish-speaking candidates are highly encouraged to apply and will be strongly preferred, as our practice serves a diverse patient population and bilingual communication is a significant asset.
