Physical Therapist

We are seeking a dedicated and experienced Physical Therapist to join our home health team. This position involves working primarily with home health patients, conducting evaluations, and developing personalized treatment plans to support recovery and enhance quality of life.

Responsibilities:

• Conduct start of care assessments, reassessments, and daily visits.
• Manage a typical workload of 5-6 visits per day, which may include admissions and assessments.
• Collaborate with a multidisciplinary team to ensure a comprehensive approach to patient care.
• Provide education and support to patients and their families regarding treatment plans and recovery processes.
• Complete documentation of patient visits and treatment progress in a computerized charting system.
• Coordinate with physical therapists in sister agencies for support and resources.

Qualifications:

• Current Physical Therapy License in North Dakota.
• Basic Life Support (BLS) certification required.
• Previous experience in home health and OASIS training is preferred.
• Must possess a valid Driver's License and auto insurance.
• Willingness to work Monday through Friday, from 8 AM to 5 PM.

Work Environment:

This role is focused on home health services, with the majority of visits occurring in the patient's home. Occasional evaluations or teaching visits may be conducted in hospice situations.

Orientation and Support:

New hires will undergo a short orientation period of 1-2 weeks, working closely with a preceptor for guidance and support during this transition.

Scrubs and Attire:

Staff are required to wear black, navy blue, or grey scrubs while on duty.

Cancellation Policy:

The organization holds the right to cancel up to 4 shifts during a 13-week assignment; fewer shifts can be canceled for shorter assignments.
