Client Care Coordinator

Social Worker - LMSWThe LMSW is responsible for coordinating inpatient and outpatient mental health and substance abuse services for families with children in a homeless shelter.Duties and responsibilities include:Responsible for a small case load of high-risk families with childrenFacilitating weekly mental health/substance abuse workshops and onsite support groups for clientsKnowing community resources, making appropriate referrals to the community and other governmental agencies for services, and coordinating servicesIndependently assessing the psychosocial functioning and needs of clients to formulate and implement a treatment planWorking from a strength-based, client-centered perspective to conduct psychosocial assessments and provide psychosocial treatmentHaving knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment proceduresProviding consultation services to other staff about the psychosocial needs of clients and the impact of psychosocial problems on health care and compliance with treatmentHaving knowledge and skill in the use of computer software applications for drafting documents, data management, and trackingProviding crisis intervention and stabilization as needed, which may require some after-hours and/or weekend follow-up for emergenciesMaintaining, reviewing, and signing off on all required documentation/reports and submitting all on timeIdentifying program needs and making recommendationsPerforming related tasks as assignedExperience, education, and special requirements include:LMSW required. Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). At least two years' experience in an area of specialized social work practice.
