Compensation

Full-Time Salary: $90,000 – $110,000 Base Salary + Benefits
Paid-Per-Visit Option: $65 – $80 per visit

What You’ll Do

As a Home Care Physical Therapist, you’ll work directly with patients in their homes and communities to help them regain mobility, improve function, and maintain independence.

Responsibilities include:

• Evaluate patients and develop individualized treatment plans
• Deliver skilled physical therapy services in the home setting
• Build meaningful patient relationships through one-on-one care
• Complete documentation accurately and on time
• Coordinate schedules and communicate with the clinical support team
• Drive clinical initiatives focused on delivering exceptional patient outcomes

Qualifications

• Active New Jersey Physical Therapist License
• Medicare documentation and billing experience preferred
• Strong communication and time management skills
• Comfortable working independently
• Computer literacy
• Current CPR Certification
• Professional liability insurance
• Ability to wear PPE as needed
• Valid Driver's License
