Job Summary

Cuready Healthcare Staffing is seeking a qualified Registered Nurse - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Perform care coordination and discharge planning for acute care patients.
• Collaborate with interdisciplinary teams to develop treatment plans.
• Utilize Epic charting system to maintain accurate patient records.
• Plan and organize family meetings to discuss patient care.
• Coordinate durable medical equipment and transportation needs.
• Perform patient assessments and plan of care following InterQual criteria.

Qualifications

• Current RN license in the state of California or pending license valid upon start date.
• Minimum of 2 years acute care hospital case management experience required.
• Experience with EPIC charting system preferred.
• Ability to work weekends on a rotational basis as needed.
• Proficient in Microsoft Office applications.
• Strong knowledge of centers for Medicare and Medicaid Services regulations.
