Cardiology RN Care Manager

Help make a lasting difference in the lives of Veterans by delivering coordinated, technology-driven cardiac care within a collaborative specialty medicine team. As a Cardiology RN Care Manager, you will combine advanced clinical assessment, cardiac testing, device management, patient education, and care coordination to help Veterans understand their treatment and remain connected to essential services. This opportunity offers meaningful patient relationships, specialized cardiovascular practice, interdisciplinary collaboration, and the ability to improve care quality across both clinical and home-monitoring settings. Duties include, but are not limited to:

• Coordinate comprehensive, Veteran-centered cardiology care in partnership with patients, families, providers, and interdisciplinary team members.
• Perform and monitor electrocardiograms, cardiac stress tests, and echocardiograms while assessing Veterans throughout diagnostic procedures.
• Administer prescribed pharmacologic agents used during cardiac testing and evaluate patient responses for effectiveness and safety.
• Interrogate and adjust implanted cardiac devices in accordance with clinical protocols and individual patient needs.
• Manage remote cardiac device monitoring in coordination with the VA National Cardiac Device Surveillance Center.
• Address cardiac device alerts and recalls by collaborating with providers, surveillance staff, vendors, and device representatives.
• Educate Veterans and families on cardiac conditions, monitoring equipment, home surveillance processes, and follow-up care.
• Advance evidence-based cardiology nursing practice by sharing clinical knowledge, supporting colleagues and students, and participating in performance-improvement activities.

Travel Requirements: You may be expected to travel 25% of the time for this position to various care locations within the service area, including Community Based Outreach Clinics (CBOCs) for outreach clinic. You must possess and maintain a current, valid, unrestricted state-issued driver's license. You must provide a safe, clean driving record from the Department of Motor Vehicles (DMV) during onboarding. You must successfully complete a driver's physical examination to demonstrate medical ability to safely operate a government-owned or leased vehicle, as required under 5 CFR 930 and HR Job Aid 05-OB-010.002.
