As a Physical Therapist on our team, you'll have the opportunity to work with a dynamic and supportive staff to provide high-quality physical therapy services to our residents.

About the Role:

As a Physical Therapist, you will be responsible for:

• Evaluating and treating residents with a variety of physical therapy needs, including mobility, strength, and endurance
• Developing and implementing individualized treatment plans to achieve optimal outcomes
• Collaborating with other healthcare professionals to ensure comprehensive care
• Maintaining accurate and detailed records of patient progress and treatment plans
• Participating in quality improvement initiatives to enhance clinical outcomes and resident satisfaction
• From $55 an hour - PRN

Responsibilities:

• Conduct initial and follow-up evaluations to assess resident needs and progress
• Develop and implement treatment plans to achieve optimal outcomes
• Provide direct physical therapy services, including exercises, modalities, and other interventions
• Monitor and document resident progress, making adjustments to treatment plans as needed
• Participate in interdisciplinary care conferences to discuss resident care plans and progress
• Maintain accurate and detailed records of patient progress and treatment plans

Requirements:

• Master's degree in Physical Therapy from an accredited program
• Current State license as a Physical Therapist
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Knowledge of Medicare, Medicaid, and private insurance regulations
