Remote Screening & Social Services Specialist

Urban Health Plan is seeking a Screening Specialist to address behavioral health and social service referrals. The role includes assessing needs, initiating internal/external referrals, and escalating high-risk cases to Licensed Social Workers. The position supports patients remotely with occasional onsite needs, and includes bi-weekly trainings. The candidate will work Monday–Friday 8am–5pm with some late nights and must be bilingual (Spanish/English). #J-18808-Ljbffr URBAN HEALTH PLAN, INC.
