Medical Social Worker: Care Coordination & Advocacy
Society for Social Work and Research
Oak Street Health is seeking a Medical Social Worker to join our Georgia-based centers, supporting psychosocial needs and coordinating care for high-risk patients and their caregivers to improve health and reduce hospitalizations.
In collaboration with the interdisciplinary team, you will assess, advocate, and connect patients with community resources, navigate benefits, and provide education and goals of care across the patient journey.
