In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Boulder, CO

Job Overview Signify Health, part of CVS Health, is hiring a Part‑Time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. The role focuses on delivering comprehensive, preventive services and building trusted relationships with patients in their own homes. Responsibilities Conduct 45‑minute in‑home evaluations, including medication and medical history review, physical assessment, and any ordered diagnostic tests (e.g., diabetic eye exams, spirometry). Use an iPad and an intuitive clinical workflow for each visit; no authority to prescribe medicine or order lab tests. Visit patients across the United States, traveling locally and within licensed states as needed. Perform virtual visits when appropriate. Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, DEE, bone density screening, and others. Maintain safety and professionalism in diverse home environments, including homes with pets, second‑hand smoke, and other potential allergens. Qualifications Active, unrestricted license(s) in the coverage area; board certification required for NPs and PAs. Multi‑state licenses preferred. Anticipated weekly hours: 29.5. Ability to travel to Boulder, Grand and surrounding areas. Valid credentials and strong interpersonal communication skills.
