In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Kings, NY

Job Overview Signify Health, part of CVS Health®, is seeking a part‑time clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations. You will travel to patients’ homes across selected states, conduct a 45‑minute comprehensive assessment, and offer diagnostic and preventive services using an intuitive clinical workflow. Job Highlights The visit lasts about 45 minutes, includes a medication and medical history review, a physical evaluation, and any ordered diagnostic tests (e.g., diabetic eye exams, spirometry). No prescribing, lab ordering, or treatment plan changes are required. Responsibilities Build trusted relationships with patients during each visit. Help millions of people nationwide connect to the right care for their unique needs. Fulfill the responsibilities of a Signify Health clinician within the specified role. Travel locally and within licensed states as needed and conduct virtual visits when required. Provide Signify Health services, including diagnostic and preventive tests. Work Environment Your primary workplace is the private residence of health plan members. You must be able to safely perform duties in homes that may contain pets, secondhand smoke, or allergens. Ability to cover Kings and surrounding areas. Compensation The pay range for this role is $46.03 – $99.14 per hour. The actual base salary will depend on experience, education, geography, and other factors.
