In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Fulton & Albany, NY

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. The role involves engaging with patients in the comfort of their own homes and helping bridge gaps in care. Key Responsibilities Conduct in‑home visits lasting approximately 45 minutes, providing a medication and medical history review, a physical evaluation, and additional diagnostic tests as requested by the health plan (e.g., diabetic eye exams, spirometry). Use an iPad and intuitive clinical workflow for each evaluation; no prescribing, lab ordering, or treatment regimen alterations are required. Build trusted relationships and help patients navigate to the right care for their unique needs. Travel locally and within licensed states as needed, and conduct virtual visits when required. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products such as spirometry and diabetic eye exams. Maintain a safe and respectful working environment within private residences, accommodating households that may contain pets, secondhand smoke, or other allergens. Required & Preferred Qualifications (NPs & PAs) Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state licenses (preferred). Ability to cover primary locations: Fulton and Albany, NY. Work Environment As an in‑home clinician, your primary workplace will be the private residence of health plan members. You must be able to safely perform duties in homes that may contain pets, secondhand smoke, and other potential airborne allergens or irritants. Compensation Pay range: $46.03 – $99.14 per hour.
