In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Suffolk County, New York

Job Overview Signify Health, a part of CVS Health, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. The clinician will visit members in their homes to conduct medication and medical history reviews, physical evaluations, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) as ordered by the member’s health plan. The evaluation takes approximately 45 minutes and is performed with a clinical workflow app on an iPad. Clinicians are not required to prescribe medication, order lab tests, or alter current treatment regimens. Responsibilities Conduct in‑home evaluations in the member’s residence. Perform medication and medical history reviews, physical examinations, and diagnostic tests as ordered. Use an iPad and intuitive clinical workflow for each evaluation. Provide virtual visits as needed. Offer Diagnostic & Preventive Services products (e.g., spirometry, DEE). Travel locally and within licensed states, where necessary. Required Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Participation in the Signify Health clinician network. Ability to work safely in private residences, which may include pets, secondhand smoke, or allergens. Preferred Qualifications Multi‑state license(s). Experience with In‑Home Health Evaluations.
