In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Monroe, Rochester, NY

Full‑time Clinician (Nurse Practitioner or Physician Assistant) Signify Health, part of CVS Health®, is seeking a Full‑time Clinician to provide In‑Home Health Evaluations to health plan members. The visit lasts about 45 minutes and includes medication & medical history review, physical evaluation, and optional diagnostic tests. Job Highlights 45‑minute in‑home evaluation with iPad workflow; no prescribing or ordering lab tests. Flexible travel within licensed states and ability to conduct virtual visits as needed. Work as part of a national network of clinicians dedicated to improving health outcomes. Provide Diagnostic & Preventive Services such as spirometry, diabetic eye exams, DEE, etc. Responsibilities Build trusted relationships to improve health outcomes and connect members to appropriate care. Conduct in‑home health evaluations as outlined above. Collaborate with Signify Health clinicians and fulfill role duties. Travel locally and statewide as required; perform virtual visits when necessary. Work Environment Clinicians perform primarily in members’ homes, which may include pets, secondhand smoke, or allergens. Candidates must be comfortable and safe working in these settings.
