In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Colorado Springs

Full-time Clinician (Nurse Practitioner or Physician Assistant) Signify Health, part of CVS Health®, is seeking a full‑time clinician to provide in‑home health evaluations, engaging with patients in the comfort of their own homes and helping bridge gaps in care. Responsibilities Conduct approximately 45‑minute in‑home visits, including medication and medical history review, a physical evaluation, and additional diagnostic tests as ordered by the health plan (e.g., diabetic eye exams, spirometry). Use an iPad with an intuitive clinical workflow for each evaluation. Provide all Signify Health services indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE). Join a national network of purpose‑driven clinicians dedicated to improving health outcomes. Assist in visiting millions of people nationwide with the goal of connecting them to the right care for their unique needs. Work with Signify Health as a clinician colleague, fulfilling the specific role requirements. Be flexible to travel locally and within licensed states as needed. Conduct virtual visits as required. Qualifications Active, unrestricted license(s) in the coverage area(s). Board certification (required). Multi‑state license(s) (preferred). Ability to cover the primary location: Colorado Springs. Clinical experience in nursing or physician assistant practice. Preferred Qualifications Experience with in‑home care or telehealth services. Work Schedule Full‑time, 40 hours per week. Pay Range $87,035.00 – $187,460.00 (base hourly rate or base annual full‑time salary; actual offer depends on experience, education, geography and other factors).
