Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling options design your own workweek

Key Responsibilities:

• Complete annual home health risk assessments in patients residences
• Evaluate comprehensive medical history, current conditions, and medication use
• Deliver education to members about their health conditions and medication regimens
• Identify potential gaps in care and offer recommendations for resolution
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner licensure in New Mexico
• Current CPR certification (BLS)
• Board Certification via AANP or ANCC required
• Minimum of 6 months' experience as an NP preferred
• Master of Science in Nursing (MSN) from an accredited program

This position offers $80-$90 per completed evaluation, with a mileage stipend and additional compensation for administrative time, rewarding your expertise and commitment to quality care.
