Responsibilities
- Conduct utilization management reviews to determine medical necessity of services and optimize patient outcomes
- Analyze records and documentation to support appropriate levels of care and discharge planning
- Collaborate with physicians, nurses, social workers, and payers to coordinate care across transitions
- Monitor documentation, coding, and regulatory compliance for accuracy and completeness
- Participate in rounds and provide floor coverage as needed to support patient care and throughput
- Facilitate efficient care transitions, including discharge planning and post-discharge follow-up as assigned

Requirements
- Active RN license to practice in New Hampshire (or eligible)
- Nursing degree from an accredited program (ADN or BSN)
- Experience in utilization management, care coordination, or a related setting is preferred
- Ability to work effectively in a skilled nursing facility environment
- Strong communication and collaboration skills to work with interdisciplinary teams
